Catherine is a 71-year-old female who admitted to The Willows at Ramapo Rehabilitation and Nursing Center with a diagnosis of congestive heart failure, exacerbation, cellulitis, and urinary tract infection. Catherine has a past medical history of respiratory failure, anemia, hypertension, and pulmonary hypertension. Catherine currently lives at a local assisted living community in which she plans to return to.

Catherine was assessed by the nursing, occupational therapy, and physical therapy teams. With all the information obtained in the assessment, a plan was made to assure a successful return home. Therapy focused on her daily living skills, ambulation, and overall strengthening. Upon her admission, she needed assistance with morning care such as getting dressed. She would find it difficult to get through morning care without fatigue and shortness of breath. Through her diligent work with her occupational therapist, she was not only able to get through morning routine with less shortness of breath and fatigue, but she was also able complete her morning routine independently!

When Catherine arrived to The Willows, she was only able to walk 50 feet with a rolling walker and moderate shortness of breath. She had a decrease in oxygen saturation and required close monitoring by the clinical team for safety. As time went on throughout Catherine’s stay, she was able to progress to walking 150 feet with a rollator with very minimal shortness of breath, and no assistance from the therapist. Additionally, due to her current diagnosis, her exercises would focus on strengthening her cardiac and pulmonary status.

As her discharge date was approaching, Catherine progressed more and more. She was encouraged to follow up with her specialists and to carry over what she has learned in her time in therapy. Upon her discharge from The Willows, Catherine had successfully met the goals set by all of her therapists. She was able to return home to her assisted living community with the tools, tips, and strategies for a successful, functional, and healthy life. We all wish Catherine the best of luck!
