Details of Experience:
Susan is a 77-year-old woman who came to The Willows on November 30, 2020. She lives with her devoted husband, but had experienced a fall at home and has a history of multiple strokes.

Upon her arrival to The Willows, Susan was greeted by the Concierge, Yehuda. She was introduced to the nursing team and was given a thorough evaluation. She was then evaluated by the therapy team, including speech, physical and occupational therapists.

The teams formed their goals and plans for getting Susan back home to her husband, as soon as possible. The physical therapy team was focused on getting Susan back to an independent level. When she arrived she was only walking 10 to 15 feet with a rollator. When she left she was able to walk more than 200 feet.

The occupational therapy team worked on building up Susan’s upper body strength and activities of daily living (ADL’s). When she arrived at The Willows, she needed maximum assistance to walk to the bathroom. When she left she was independent. When she was discharged from The Willows, she was able to do steps with standby assist only. Upon arrival, she also needed maximum assistance for transfers, and when she left she only needed supervision. Her therapists said her biggest hardship was her focus and left side neglect.

Susan was visited daily by the Concierge, Yehuda. She always had a nice word for everyone, always had a smile on her face, and was very thankful. Her husband came every single day and visited her by her window.

Upon her discharge, Susan returned to her home and her devoted husband. She will continue to require outpatient therapy for cognitive function. The therapists are confident that she will do great at home.
