How did this patient hear about Ramapo Manor? Referred to Ramapo Manor by Michelle the case manager at Nyack Hospital.
Patient came to Ramapo Manor on July 29th 2015 after a 10 day stay at GSH. She had been battling Pneumonia and also has COPD. Luckily for her, we had just rolled out a brand new Pulmonary Rehabilitation Program here at Ramapo Manor. Pulmonary rehabilitation is indicated for any respiratory patient who, despite otherwise optimal medical therapy remains symptomatic, has decreased exercise tolerance, is impaired in the ability to perform activities of daily living, or has decreased quality of life.
The initial assessment for pulmonary rehabilitation is performed by the appropriate program personnel under the direction of the program coordinator and medical director. The initial assessment identifies specific needs and problems, sets the foundation for all subsequent services provided by pulmonary rehabilitation, and allows for the development of an individual plan of care. Realistic patient goals are determined during the assessment and reevaluated during the program as necessary. A thorough assessment is the cornerstone in optimizing safety and effectiveness of pulmonary rehabilitation. Bernice was a perfect candidate for the program and she started it a few days after being at Ramapo Manor. Prior to being a participant in this program she was getting 5 liters of oxygen daily. Since entering the program each day her breathing was getting stronger and she was able to walk farther and farther. After three weeks in the program she was walking without any oxygen. For an 87 year old woman who has been battling COPD for many years this was such a wonderful thing to see. You would see her in the hallways and she would be glowing with happiness. After speaking with Joe Potts our Head Respiratory Therapist he said to me, “Kris this has truly been a miracle. This just goes to show you that this program works for anyone who is battling breathing issues. I am so proud of her for the strides she made and the hard work she provided while in this program.†Patient is now able to go home and get back to her everyday living. This was such a success story and I was so honored to be a part of this and witness it all.
